At a Glance Legacy Care is currently recruiting a Transitional Care Management (TCM) Nurse Practitioner with an active Florida license.  This is a part-time position in an acute/home/telehealth environment.  Legacy Care is a nurse and physician-led medical group of physicians and advanced care practitioners who deliver quality care to patients in post-acute, long-term care, inpatient rehabilitation and assisted living communities across the nation.  We are devoted to ensuring a patient’s healthcare experience meets their individual needs across the continuum of care. Essential Duties and Responsibilities In one or more of the acute, home, and telehealth environments, the TCM Nurse Practitioner will provide the following: - TCM/Advanced Care Planning patient visits - Collaborate with Case Management and acute teams (in acute setting) - Establish transitional communication post-acute facilities if applicable - Devise, document and maintain a transitional care plan - Communicate with patient and family regarding transitional care plan - Refer to ancillary services as needed - Collaborate with PCP and specialty care teams - Ensure continuity across all care levels during the transition period - Document visit and enter coordinating charges - Maintain communication with collaborating physician - Deliver superior care while maintaining all company metrics pertaining to productivity - Serve as the patient resource for transitional care information and follow-up care coordination during the transition period - Other duties as requested or assigned.

Summary The FP&A will be an integral part of the organization and report directly to the CFO of the LGS Management Group. The FP&A will prepare internal financial reports, monthly financial analysis materials, preparation tasks for Board of Director meetings, discussion and analysis of financial results and provide ad hoc analysis for the multi-company organization. They are responsible for providing senior executives with the analysis and information they need to make major operational, financial, and strategic decisions. The FP&A will assist in the annual budgeting process, managing cash flow forecast models, variance analysis, and other financial performance tools as needed.   Essential Duties and Responsibilities - The "eyes and ears" of the organization; central liaison between corporate and operations teams. As such, FP&A will interface with the CFO as well as the CEOs on a daily basis - Use all available tools and data bases to generate and analyze current and past trends in key performance indicators including all areas of revenue, sales, expenses and capital expenditures - Assess and evaluate financial performance of organizations with regard to short and long-term operational goals, budgets and forecasts - Utilizes forecasts and variance data to advise management on decisions such as, how to improve performance, minimize risk or capture new opportunities from both within the company and within the external environment - Assist in identifying, acquiring and implementing systems and software to provide critical financial and operational information - Monitor performance indicators established by management, highlighting trends and analyzing causes of unexpected variances - As a key part of the forecasting process, he/she will conduct variance analysis to show executive leadership how the budget and/or rolling forecast compares against actual performance - Ability to connect senior management's long term "strategic plan" to reality; Develop financial models to support strategic initiatives - Analyze complex financial information and reports to provide accurate and timely financial recommendations to management for decision making purposes - Ability to develop a rolling forecast, either in parallel with the traditional budget or altogether as a replacement
