We are currently recruiting a Nurse Practitioner with active Virginia NP and DEA license for a full-time position providing patient consultation and education in the transition of care from skilled nursing facilities to home for the purpose of mitigating hospital readmissions.

In this role, your responsibilities will include:

  • Researching and understanding the patient's condition and the treatment plan
  • Providing purpose, education and expectations in a consultation with the patient
  • Encouraging follow through with a primary care provider
  • Charting, entering charges

Qualifications

Qualifications

  • Nurse Practitioner degree from an accredited nursing school
  • Active Virginia NP license and DEA license in good standing
  • 2 years of experience as a Nurse Practitioner ( 1 year post-acute care experience preferred)
  • Proficiency with EMR 
  • Compassionate, Intelligent, Motivated, Team Player
